The following forms may be downloaded and printed and may be submitted via hand-delivery, U.S. mail, or scanned and sent to the Judicial Affairs office via e-mail using portable document format (PDF).
Approved Assigned Community Service Hour Organizations: If you have been given permission by Judicial Affairs to complete your assigned community service hours at any of the Judicial Affairs approved organizations, here is the list.
Assigned Community Service Hours Record Form: This form is for students who have been asked to complete assigned community service hours. Students must use this form for submitting completed hours.
Request to Review Judicial Affairs Disciplinary Record: If you wish to review your disciplinary record, you will need to complete this form. You must complete and submit this form and then schedule an appointment with the Judicial Affairs office in order to review your file.
Consent To Disclose Judicial Affairs Disciplinary Record: If you wish for Judicial Affairs to release information from your disciplinary file to someone other than yourself, you must complete this form.
Petition to Expunge Judicial Affairs Disciplinary Record: If you would like to request that Judicial Affairs expunge your disciplinary file prior to seven years from the year in which it was created, you must complete this form. The Associate Dean for Judicial Affairs will review the request and make a decision.

Reflection Journal: This form is for students who have been asked to submit a reflection journal. There are 5 questions on this form to which students are asked to respond. Completed reflection journals may be e-mailed to Judicial Affairs.
